Go Like the Wind Montessori School (“GLTW” or “the school”) agrees to enroll / re-enroll the above student for the Infant/Toddler program. In consideration of acceptance of this Enrollment Agreement (“Agreement”) by GLTW, I agree to abide by the listed conditions and pay the required tuition and fees as specified below. This Agreement is not effective until fully executed by the parent(s) or guardian(s) and GLTW.

GLTW utilizes a continuous enrollment process. Once a child is enrolled, they will automatically be enrolled every year at the same level of participation unless or until GLTW is notified otherwise by the parent.

I understand that the Infant and Toddler programs are five days per week programs and follow an ongoing year-round schedule.

I understand that I will be solely responsible for meeting the financial obligations set forth in this agreement. Any financial assistance for the child must be arranged and maintained by the undersigned.

I understand that I will be responsible for providing food, milk, formula, diapers, wipes and other needs for my child.

GLTW requires a $1,200.00 deposit to be paid upon submission of the application. GLTW requires that all families make a six (6) month commitment to the program, if a child is withdrawn before the six (6) month period, the deposit will be forfeited. After six (6) months of enrollment, the deposit will be refunded in full upon thirty (30) days written notice of withdrawal and after all outstanding balances are paid in full. If the child transfers to the Primary program, the deposit will be applied to the tuition of that program.

The Application Fee of $75.00 is separate from the Program fees and deposit. The Application Fee must be paid at the time of submission of the application in order to reserve a space for the child. TheApplicationfeeisnon-refundable.

I understand that GLTW must be notified in writing thirty (30) days in advance of any intended changes to attendance, schedule or withdrawal date. A notice of withdrawal must include the reason for withdrawal. Fees for the program will be due and payable until such written notice is received. Fees are due for thirty (30) days after the written notice is received. GLTW will calculate any refunds or balances due based on the date of receipt of the written notice to withdraw plus the thirty (30) day period. Withdrawal for any reason forfeits a guaranteed spot in the program and places the child at the end of any active wait list.

A child whose fees are thirty (30) days past due may not attend the program until the fees are up to date without the special permission of the Administrator. GLTW will not release any records for a transferring child until all open balances have been paid. GLTW will not allow re- enrollment of the child for any school program until all past due fees are paid in full. Accounts that are sixty (60) days past due will be referred to collections.

There is no tuition credit or deduction for absences (due to illness, vacation, or other reason), closure for inclement weather, school holidays, other events that necessitate closing the school that are beyond the control of GLTW, or administrative or teaching staff changes.

The undersigned further agrees to participate in a minimum of 5 hours of Family Service Hours to the school during the term of the contract. These hours include assisting with concerts and PTSA events.

The school maintains a licensing notebook of all licensing inspection reports, special investigation reports and all related corrective action plans. The notebook will be available to parents for review during regular business hours. Licensing inspection and special investigation reports from at least the past two years are available on the Bureau of Children and Adult Licensing website at www.michigan.gov/michildcare.

GLTW requires the cooperation and support of the parents and guardians of the student to successfully provide the Montessori educational experience. I will support the aims and ideals of the school and will bring any and all questions, in confidence, first to the appropriate teacher, and, if necessary, to the Administrator. The absence of cooperation and support by the parents or guardians, at the discretion of GLTW, is cause for termination of this Agreement.

This Agreement shall be interpreted in accordance with the laws of the State of Michigan.
